The drastic increase of self-neglect, abuse, non-scheduled, missed, and no-show appointments has necessitated the need for our case management and services. NHCCBS, LLC provide centralized, efficient client friendly system to reduce relapses/ readmission and prevention. 

 Our, turnkey, program provides services, which allows for reducing stress, effective and efficient services by permitting faster and smooth-running services for our clients. Our simplified, proficient scheduling system is designed to bridge the gap between the client and their provider(s). We schedule initial appointments, follow-up appointments (rather than unnecessary and inappropriate appointment scheduling), phone call reminders, handle in-bound and out-bound calls from providers to our clients. NHCCBS, LLC also provide Client Watch Services  in assisting with client safety, happiness and comfortability.

NHCCBS, LLC owner and staff have many years of experience with customer/patient care, risk management, and care management. Our services are flexible and affordable. We have the knowledge and expertise required to customize the service needs of our clients. 

 Our call center helps improve care coordination by performing partnerships with your providers. NHCCBS, LLC services also lead to exceeding quality and safety of our clients. Our system expedites services to our clients and provides a customer-centered model toward continuity of care. We arrange, monitor, and provide referrals.  

 Our Program

NHCCBS, LLC  is able to identify risk factors that can result in imminent safety concerns for our clients, which results in State Agency (i.e., APS, Law Enforcement and the courts) involvement. NHCCBS, LLC  also provides you and your family with the security, of knowing your loved one is safe. This service provides a benefit of monitoring clients for abuse, neglect, and exploitation.

The rationale for developing this program descends from the belief that providing optimum care for clients is not a singular event, but rather a never-ending process. The Our Program was created to monitor and safeguard that providers are providing appropriate, efficient, timely, and safe services, per the States standard of quality care for at the risk populations. 

Physical, mental, emotional, neglect, and exploitation are common types of abuse perpetrated on this population. According to Elder Abuse: Report Elder Abuse and Nursing Home Neglect in the U.S. alone, a large population, “more than half a million reports of abuse against” (n.d.), individuals with Limited Mobility, Intellectual Disabilities, Behavioral Delays, and the Elderly Population are reported to authorities each year, and “millions more cases are unreported.”

Currently, individuals with Limited Mobility, Intellectual Disabilities, Behavioral Delays, and the Elderly Population are frequently abused, neglected, and exploited in their homes and communities. For these reasons, NHCCBS, LLC created a program to ensure the safety of the most vulnerable.

We advocate for our clients and ensure that their basic needs are being met. We provide scheduled and unannounced visits to our clients, placed in group homes, assisted living facilities and other placements. We also strive to ensure clients are treated respectfully in their placements and are not being abused, neglected, or exploited.
